Billionhair Network: Social Security Number and Individual Taxpayer Identification Number Notice

Why does the Company need my Social Security Number (SSN) or an Individual Taxpayer Identification Number (ITIN)?
Federal tax law mandates that the Company requests a valid SSN or ITIN from Billionhair Network Independent Beauty Consultants who earn commissions, prizes, awards, or gifts from the Company.

When should I submit my SSN/ITIN?
To avoid federal and state backup withholding, submit your SSN or ITIN before earning commissions, prizes, awards, or gifts.

What happens if I choose not to provide my SSN/ITIN prior to earning a commission, prize, award, or gift?
If you choose not to report your SSN or ITIN to Billionhair Network Inc., the Company is required to withhold federal income tax (backup withholding) at a rate of 24 percent plus applicable state backup withholding on income earned (commissions, prizes, awards, and gifts). In addition to backup withholding, you may be subject to a $50 penalty by the IRS for failing to provide your SSN or ITIN.
